This form must be completed and on file along with the Medical/Liability Release Form in order for your child to participate in Camp Totus Tuus.

Every camper who attends Camp Totus Tuus must bring with them a completed and signed medical release form.

Totus Tuus of Wichita provides partial scholarships for campers from the Diocese of Wichita whose families are experiencing financial difficulties.  We want everyone who wants to go to camp to be able to go to camp, and we don't want cost to inhibit anyone from going.  However, we have only a limited amount of scholarship money available.  Therefore, if you feel you qualify, please complete this application and return it as soon as possible to the Totus Tuus office.  Application deadline: May 1, 2012.
