Medical Release/Permission Forms must be filled out clearly & completely and signed by the youth and their parent or guardian. Diocesan medical release forms are required for Conference, TEC, CLAY, Summer Youth Leadership Day and other Diocesan sponsored events. Medical release forms should be kept on file for a minimum of two years in a safe place and should be shredded at the end of those two years. Diocesan medical release forms may be used and/or adapted for all retreats, lock-ins, mission trips, conferences, summer trips, ski-trips, sporting events etc. If you have any questions about the Medical Release Form please contact the Office of Youth & Young Adult Ministry at (316) 269-3930.
